We are seeking 2 x Assessment Officers for a local Council in the Western Suburbs. Use your experience as an Assessment Officer to facilitate HACC Services for older people and people with disabilities, to enable them to live independently in their own homes.

This is a short term assignment with a minimum 4 weeks, Full Time.
Ideally you will have the following:
• Tertiary qualification and/or relevant experience in Nursing, Allied Health, Case Management or Social Services. • Knowledge of HACC National Standards and Guidelines.
• Experience in assessment, care planning and case management.
• Demonstrated knowledge and understanding of the care issues associated with the aged and people with disabilities
• Experience in the operation of a computer, including use of a lap top in the field, database maintenance, report writing and spreadsheets essential.
• Ability to work and communicate with people from diverse socio-economic and ethnic backgrounds across a broad age range.
• Current Victorian drivers license
• Pre – employment Police Check/Working With Children Check
• First Aid- Level 2
• A community language would be an advantage.
• Experience with Expedite an advantage
• Experience in a Council Setting
